Highlights • Lifestyle interventions may decrease or even eliminate the short-term need for glucose-lowering medications in persons with short-standing type 2 diabetes.• What are the long-term effects of a 1-year lifestyle intervention aimed at reducing glucose-lowering medications on glycemic control and cardiometabolic risk factors in persons with a short duration (<10 years) of type 2 diabetes?• The benefits of a 1-year lifestyle intervention on HbA1c and cardiometabolic risk factors are not sustained 5 years after the 1-year intervention, and standard care is associated with more favorable diastolic BP control.• The management of non-glycemic risk factors should receive increased attention for persons living with type 2 diabetes who aim to achieve lifestyle-induced reductions in glucose-lowering medications.
