BACKGROUND Pediatric patients with chronic kidney disease (CKD) undergoing maintenance hemodialysis frequently exhibit compromised physical performance and reduced quality of life (QoL). The present study aimed to examine the effects of an eight-week combined intradialytic exercise intervention on QoL, functional capacity, and dialysis adequacy, defined as an index of urea clearance efficiency during hemodialysis, in this patient population. METHOD In a quasi-experimental, one-group, before-and-after trial, five children (aged 8-18 years) with CKD on maintenance hemodialysis completed an eight-week intervention. The program, conducted three times a week during dialysis, consisted of upper-body resistance exercises (two sets of 10 repetitions at the 10-repetition maximum) and aerobic cycling (10-15 min, with intensity monitored via the Children's OMNI Scale). Outcomes included QoL (Pediatric Quality of Life Inventory [PedsQL™] 4.0, child and parent reports), functional capacity (handgrip strength and 60-s sit-to-stand [STS60] tests), and dialysis adequacy (single-pool Kt/V). RESULTS Child-reported PedsQL™ scores significantly improved for total QoL (Δx̄= 6.80, 95% CI [1.01, 12.58], p = 0.031, d = 1.46), emotional functioning (p = 0.043, d = 1.30), and social functioning (p = 0.042, d = 1.32). Handgrip strength (Δx̄= 2.20 kg, 95% CI [1.25, 3.14], p = 0.003, d = 2.90) and STS60 performance (Δx̄= 2.80 repetitions, 95% CI [0.57, 5.02], p = 0.025, d = 1.56) also increased signi...
