Clinical and anatomical profile of patients undergoing cryoballoon left common pulmonary vein isolation using the "trefoil" technique: a descriptive analysis of a successful intervention cohort

Aim . To provide a detailed description of the clinical, echocardiographic, and computed tomography (CT) anatomical characteristics of patients with verified clinically successful left common pulmonary vein (LCPV) isolation using the "trefoil" technique, and to develop practical guidelines for preoperative candidate selection for this procedure. Material and methods . This single-center descriptive study included 85 patients with clinically effective LCPV isolation using the "trefoil" technique, selected from a cohort of 98 patients with atrial fibrillation (AF) and an LCPV diameter >28 mm. We analyzed clinical characteristics, echocardiographic indices, and anatomical parameters based on left atrial and LCPV computed tomography. Quantitative data are presented as mean±standard deviation or median and interquartile range, depending on the distribution pattern. The criterion for success was defined as freedom from AF recurrence during the follow-up period or confirmed persistent LCPV isolation based on repeat endocardial electrophysiology study in patients with arrhythmia recurrence. Results . The study cohort was dominated by patients with paroxysmal atrial fibrillation (68,2%), with a mean age of 59,9±10,3 years.
