Abstract: Long COVID has emerged as a continuing public health challenge, with symptoms such as fatigue, cognitive slowing and shortness of breath affecting many people well after the initial SARS-CoV-2 infection. This review synthesizes evidence on prevalence, symptom, risk determinants, health utilization, and the wider social and economic effects of Long COVID, emphasizing prevention and health system preparedness. The analysis highlights differences across regions and population groups, rising pressure on rehabilitation services and notable consequences for employment, education and mental health. The study looks at fairness in resource distribution within overburdened health systems, equity in care access, and the protection of vulnerable groups in research and policy responses from an ethical standpoint. We underline the importance of harmonized diagnostic criteria, inclusive and representative research, and equitable access to chronic care services. We also set out priority policy actions to support earlier identification, team-based models of care and coordinated national strategies. By integrating clinical, economic and social perspectives, this review outlines a policy priorities to support early identification, multidisciplinary care, and coordinated national strategies. We also examine bioethical considerations including equity in care access, resource allocation in strained health systems, and protection of vulnerable populations in research and policy responses.
