Research Progress on the Theoretical Connotation of Blood-Heat Syndrome and Its Immuno-Inflammatory Mechanisms in Children with Allergic Rhinitis

儿童过敏性鼻炎属中医学“鼻鼽”范畴,临床除传统虚寒证型外,血热证型日益受到关注。本文基于《中医临床诊疗术语 第2部分:证候》(GB/T 16751.2-2021),梳理血热证总纲及五亚型的理论内涵,探讨血热致鼻鼽的中医病机路径;从IgE介导的致敏机制、Th2免疫偏移、氧化应激等维度分析其免疫学基础,并引入犀角地黄汤、白茅根、黄芩等清热凉血方药的现代药理研究作为佐证,以期为儿童过敏性鼻炎的中西医结合防治提供参考。Allergic rhinitis (AR) in children falls into the category of Biqiu (nasal congestion) in traditional Chinese medicine (TCM). Apart from the conventional deficiency-cold syndrome patterns, the blood-heat syndrome pattern has increasingly drawn clinical attention. Based on Clinical Terminologies of Traditional Chinese Medicine—Part 2: Syndromes (GB/T 16751.2-2021), this article reviews the theoretical connotation of the general framework and five subtypes of blood-heat syndrome, and explores the TCM pathogenetic pathways of blood-heat inducing Biqiu. It further analyzes the immunological underpinnings from the perspectives of IgE-mediated sensitization mechanisms, Th2 immune deviation, and oxidative stress, supported by modern pharmacological evidence of heat-clearing and blood-cooling formulae such as Xijiao Dihuang Decoction, Bai Maogen, and Huangqin, aiming to provide a reference for the integrated Chinese and Western medicine prevention and treatment of this disease.
