Summary A recent article presents the largest histogenomic characterization of first-line osimertinib resistance from the ORCHARD study. Dual-modality tissue and plasma sequencing detected resistance alterations in 87% of patients and polyclonal mechanisms in 46% of patients, highlighting the complexity of post-osimertinib biology and raising questions about targeted versus broadly acting regimens in the post-EGFR tyrosine kinase inhibitor setting.
