Advanced hydrogel platforms for inflammatory bowel disease therapy: Smart drug delivery, immunomodulation, and remodeling of the gut microenvironment

Inflammatory bowel disease (IBD) is a chronic inflammatory disorder of the gastrointestinal tract with rising global incidence, significantly impairing patients' quality of life and posing life-threatening risks in severe cases. Recent advances in understanding the pathogenesis of IBD have spurred exploration into hydrogel-based therapeutic strategies, owing to their unique physicochemical properties and biocompatibility. This review systematically examines the innovative applications of hydrogels in IBD treatment through a mechanism-driven classification framework, encompassing stimuli-responsive drug delivery systems, mucosal barrier restoration, immunomodulation, gut microbiota modulation, and multifunctional integrated platforms. This review aims to provide insights into the rational design of next-generation hydrogels with enhanced therapeutic efficacy and multifunctionality for inflammation alleviation by analyzing current research progress. Furthermore, we critically discuss the translational challenges and potential limitations of hydrogel-based therapies, including biocompatibility, scalability, and clinical applicability. Ultimately, this review seeks to guide the development of advanced hydrogel materials, offering a novel and effective paradigm for IBD management.
