Lutein, an important carotenoid, has traditionally been recognized for its well-established benefits in eye health. Building on a review of its classic protective functions for vision, this review further explores its emerging roles in brain health, skin photoprotection, and metabolic regulation. We have collected evidence from clinical observations, in vitro studies, and animal models, revealing the significant potentials of lutein in protecting cognitive function, maintaining hippocampal integrity, modulating neuroinflammation, alleviating skin photodamage, and regulating insulin sensitivity via the gut microbiota-host metabolism axis. The review particularly highlights that current research remains largely in the preclinical stage and delves into the existing translational gap between basic discoveries and clinical applications. Finally, lutein is discussed within the broader context of nutritional networks, and future research directions, especially the need for high-quality human clinical trials to validate its clinical efficacy, are outlined, aiming to provide a balanced and critical perspective on the comprehensive health benefits of lutein as a multifunctional nutrient.
