Summary Adhesive hydrogels have potential as next-generation suture replacements in wound closure. However, existing products perform poorly in hemostasis for extreme bleeding and tissue regeneration. Herein, a body fluid-triggered adhesive hydrogel was prepared. This hydrogel can repel the liquid membrane through temporary phase separation, enabling the adhesive molecules to contact the amino groups directly on the tissue surface, form covalent bonds, and adhere firmly without additional intervention. The excellent performance of this hydrogel has been validated in rats, large animals, and humans. It can achieve rapid hemostasis within 30 s for extreme blood loss caused by transection of the pig femoral artery without blocking blood flow. Furthermore, this hydrogel can rapidly seal irregular lung wounds and prevent air leakage while inducing the formation of alveolar-like structures. Excitingly, the hydrogel has been applied in real-world clinical patients and demonstrated excellent efficacy. This hydrogel provides a new approach to rapidly seal complex wounds and regenerate tissue.
