Predictable Restoration Of Maxillary Anterior Residual Roots Through Digital Workflow

Restoring maxillary anterior teeth with residual roots poses significant clinical challenges. Digital integrated design and fabrication of post-and-core crowns offer an effective solution for achieving aesthetic and functional rehabilitation. A 55-year-old female patient presented with residual roots at teeth #12–23 and prepared abutments at #14, 13, and 24, after removal of her existing prostheses of maxillary anterior teeth. Following root canal therapy and crown lengthening, multimodal datasets—including facial scans, intraoral scans, CBCT imaging, and mandibular movement records captured via electronic facial arch—were acquired, analyzed, and digitally integrated. Guided by a digital workflow, custom cast post-and-core systems (with tooth axes changed) and provisional crowns were simultaneously fabricated and placed. After six months of healing and functional evaluation, definitive zirconia crowns were delivered, achieving optimal aesthetic outcomes and harmonious occlusal function during protrusive and lateral movement. Adhering to an end goal-driven philosophy, the treatment prioritized patient-centered full-crown prosthetic design, followed by reverse-engineering of post-and-core systems, while comprehensively risk evaluating and optimizing material selection.
