Higher sexual frequency correlated with lowered PSA complex levels in U.S. men aged 40–59 and without prostate conditions: a nationwide cross-sectional study

Prostate-specific antigen (PSA) remains a critical biomarker for prostate-related conditions, including benign prostatic hyperplasia, prostatitis, and urinary tract infections.1 However, factors influencing PSA levels in men without prostate diseases are underexplored. This study examines the relationship between sexual frequency and PSA levels among U.S. men aged 40–59 without prostate conditions, utilizing data from the National Health and Nutrition Examination Survey (NHANES) 2007–2010. Data from 942 participants aged 40–59, representing 26 263 733 of American population, were analyzed, see Supplementary Figure 1. Sexual frequency was self-reported via questionnaires, while total PSA (tPSA), complexed PSA (cPSA), and free PSA (fPSA) levels were measured from serum. Covariates for analyses were selected based on previous studies.2,3 As illustrated in Supplementary Table 1, participants’ tPSA concentration was 0.97 ± 1.02 on average, and decreases with increasing sexual frequency (“Never”: 1.14 ± .90; “1-11 times”: 1.03 ± 1.25; “12-51 times”: 1.00 ± 1.15; “52-103 times”: .87 ± .57; “104-364 times”: .81 ± .49; “>= 365 times”: .62 ± .73; P = .028). A similar inverse correlation was also observed in cPSA levels with sexual frequency. However, no significant correlation was found between fPSA levels and sexual frequency (P = .2).
