Clinical characteristics and predictive models of plastic bronchitis caused by mycoplasma pneumoniae pneumonia in children

BACKGROUND: The prevalence of mycoplasma pneumoniae pneumonia (MPP) in China in 2023 is accompanied by an increase in cases of plastic bronchitis (PB), which provides an opportunity to explore its risk factors. METHODS: A retrospective analysis of 282 children with MPP who underwent fiber-optic bronchoscopy (FOB) at our institution was performed. Based on the results of FOB, the participants were divided into two groups: PB and non-PB. The basic information, clinical manifestations, treatments, laboratory results, and chest imaging findings of the two groups were compared. For model construction, 200 children were selected for the training set, while 82 children were selected for external validation in the validation set. Least absolute shrinkage selection operator regression and various cross-validations were used to select potential variables. Multivariate logistic regression analysis is used to structure nomograms. Use ROC curves, Akaike information criterion, and calibration curves to screen and evaluate the model. RESULTS: Multivariable logistic regression analysis identified risk factors for PB caused by MPP, including age, peak body temperature, atelectasis, low serum albumin, and lactic dehydrogenase. We have developed a nomogram based on this. The area under the curve of the nomogram was 0.8768 (95 % CI: 0.8206-0.933) in the training set and 0.8399 (95 % CI: 0.7428-0.9371) in the validation set, demonstrating good discriminative ability and accuracy.
