Recently, the combination of transarterial intervention therapy (transcatheter arterial chemoembolization [TACE] or hepatic arterial infusion chemotherapy [HAIC]) and systemic therapy for unresectable hepatocellular carcinoma (HCC) has yielded controversial results. In this study, we aimed to evaluate the efficacy and safety of the combination therapy. We conducted a systematic review and meta-analysis. PubMed, Embase, Web of Science, and the Cochrane Library were searched for relevant studies from their inception to March 6, 2025. Randomized controlled trials published in English were included if they compared TACE or HAIC plus tyrosine kinase inhibitors (TKIs) and/or immune checkpoint inhibitors (ICIs) with either TACE or HAIC alone or with TKIs and/or ICIs alone in unresectable HCC. Data were extracted from the included studies in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The outcomes of this meta-analysis included overall survival (OS), progression-free survival (PFS), time to progression (TTP), objective response rate (ORR), disease control rate (DCR), and adverse events (AEs). Twenty studies, including 5485 patients with unresectable HCC, were included in the meta-analysis. Upon pooled analysis, longer OS, PFS, and TTP were observed for the combination of TACE/HAIC and systemic therapy compared with TACE/HAIC monotherapy or systemic monotherapy, with pooled hazard ratios of 0.75 (95 % CI, 0.62–0.92; P = 0.005), 0....
