Ablative fractional laser treatment for the clinical improvement of acne vulgaris: A case series

Acne vulgaris is the most common inflammatory skin condition, often resulting in scarring and pigmentary changes.1, 2 Severe nodular acne causes significant psychological distress and leads to morbidity and in rare cases, suicide attempts.3-5 The current standard of care for moderate to severe acne involves oral antibiotics, oral hormonal therapy, or oral isotretinoin.1, 4, 6, 7 However, the increasing prevalence of antibiotic-resistant bacteria2, 8 and the side effects of isotretinoin and spironolactone, coupled with challenges associated with the iPledge system, have created a need for effective alternative therapies.9 Many light and laser-based approaches have been investigated for treating active acne.2, 10, 11 Recently, the FDA approved the 1726-nm laser with contact cooling for acne treatment.12 The role of the fractional carbon dioxide (CO2) laser in acne scars has already been well-defined. The energy per pulse and treatment density generally used to treat acne scars or scars, in general, is high.2, 4, 5, 13 Further, acne scar treatment by CO2 lasers is usually delayed until the active lesions are gone.7 This self-controlled study evaluated the efficacy of ablative fractional CO2 laser treatment for the clinical improvement or treatment of active inflammatory acne vulgaris.
