Background Detection of p16 overexpression using immunohistochemistry is the required test method of the current 8th edition of the TNM-classification to detect HPV-induced carcinogenesis. Due to it′s moderate specificity, however, p16 testing alone is only sufficient to a limited extent. Material and methods A total of 132 patients with the initial diagnosis of oropharyngeal carcinoma (OSCC) who were treated in the ENT clinic of the University Hospital Münster between November 2018 and October 2022 were included. The tumor tissue was analyzed for p16-overexpression using immunohistochemistry (p16 antibody, clone E6H4, Ventana BenchMark, Roche, Basel) and for HPV-specific DNA (HPVType 3.5 LCD array kit, Chipron GmbH, Berlin). The Kaplan-Meier method and the log-rank test were used for analysis of progression-free survival (PFS). results 81.8% (n=54) of the HPV-DNA-positive OSCC could detect HPV type 16, 6.2% (n=4) type 33, 1.5% (n=2) type 31 and each in 1.5% (n=1) types 6, 18, 35, 56, 58, 73 were detected in the tumor tissue. 44.7% (n=59) of OSCC were double negative for p16 and HPV DNA tests; 46.2% (n=61) were double positive; 5.3% were p16 positive/HPV negative and 3.8% were p16 negative/HPV DNA positive. Patients with a double-positive OSSC showed the most favorable PFS (3-year PFS 66.5%). PFS of cases with discrepant results on p16 and HPV DNA tests was similar to patients with a double-negative OSCC. conclusion Consequently, HPV-induced carcinogenesis should be confirmed...
