The ChinaHEART cohort is a population cohort with wide geographical coverage and a large sample size. A total of 4 404 586 participants aged 35–75 years have been recruited from all 31 provinces in Mainland China during 2014–22. The objectives include exploring unknown genetic, metabolic, behavioural and environmental risk factors and their interactions on major non-communicable diseases, as well as assessing the known risk factors’ effects and their heterogeneities in the diverse community-based population. The main categories of data collected at baseline are demographic and sociodemographic characteristics, health behaviours, history of diseases and treatments, family medical histories and quality of life. Additionally, physical measurements, laboratory tests and imaging examinations were conducted. Blood samples and urine samples were collected from 2.7 million participants, centrifuged and allocated at study sites, then stored for central laboratory tests and future research purposes. Passive follow-up of nearly 16 million person-years for mortality was conducted for all participants through the National Mortality Surveillance System and Vital Registration, and 3.3 million active follow-up visits were carried out regularly among participants with high cardiovascular risk.
