Introduction: Limited information is available about the medication use in stable COPD patients from Romania. Objective: To describe the types of respiratory medication prescribed by pulmonologists in clinical practice and explore the treatment patterns. Methods: Out of 2162 patients from a large cross-sectional, multinational, observational study (NCT03031769), 406 were from Romania. Patients with stable COPD were included, irrespective of medication. Enrolment lasted from December 2016 to August 2017. Data about COPD treatment in the last 2 months was collected. Results: Romanian group included 83.5% males with mean age (±SD) 65.9±8.5 years, mean post-bronchodilator FEV1 (±SD) 47.82%±16.05% and mean number of exacerbations (±SD) in the last year 1.97±2.68. Most patients had moderate (43.3%) or severe (40.4%) airflow obstruction. The ABCD GOLD 2017 grading revealed persistent symptoms: 86.7% of which 40.9% in B and 45.8% in D vs. 9.9% patients in A and 3.4% in C. Overall, most frequently used classes of medication included combinations of long-acting bronchodilators (LABA)+inhaled corticosteroids (ICS) (57.9%) in one inhaler, short-acting bronchodilators (SABA) (41.9%), long acting muscarinic antagonists (LAMA) (34.5%), methylxanthines (16.3%) and oxygen use (15.3%). Thirty patterns of maintenance treatment were identified, irrespective of the type of short-acting bronchodilator associated. Top 5 included: LABA+ICS (26.1%), LABA+LAMA (16.5%), LAMA alone (10.1%), LABA+...
