Treatment patterns in stable COPD patients – results from a large observational European study (SPACE)

Background: Treatment guidelines do not always reflect real-life prescribing patterns. Limited data about implementation of GOLD guidelines recommendations in clinical practice in medium and small-sized European countries exist. Aims: To describe real-life treatment patterns in a large cohort of stable COPD patients. Methods: Observational, cross-sectional, multinational study (NCT03031769) conducted in 11 countries that included 2162 stable COPD patients, irrespective of their respiratory medication. Classes of medication were collected and patterns analyzed in the overall cohort and across countries, including ABCD GOLD groups. Results: In total, 2139 (98.9%) patients received treatment of any class. Overall, the most common maintenance treatments were long-acting muscarinic antagonists (LAMA) alone (51.4%), combined long-acting bronchodilators (LABA)+inhaled corticosteroids (ICS) (51.3%) and combined LABA+LAMA (23.5%). From the 77 patterns identified, triple therapy with LAMA+LABA+ICS was the most frequently used (41.0%), irrespective of short-bronchodilators or other maintenance medication associated (e.g., mucolytics, methylxantines, etc). At country level, the Netherlands had the highest usage of triple therapy (52.9%) and Romania the lowest (9.4%). ICS in different combinations was used in all GOLD 2017 groups (A:48.8%, B:60.3%, C:66.3%, D:69.5%) including triple therapy (A:30.6%, B:38.1%, C:42.7%, D:48.6%).
