Evaluation of non-contrast MR angiography(NC-MRA) using balanced steady-state free precession(bSSFP) and flow sensitive dephasing(FSD) for detecting peripheral arterial disease in patients with diabetes

Objective:To evaluate NC-MRA using SSFP and FSD in the detection of lower-extremity arterial disease in patients with diabetes.Methods:Forty-two patients with diabetes underwent bilateral distal lower extremity NC-MRA before clinical routine contrast-enhanced MRA(CE-MRA) in the same session.The lower-extremity arteries include three segments:anterior tibial artery,posterior tibial artery and peroneal artery.Two experienced observers who were blinded to the result of NC-MRA on a four-point scale and graded the significant stenosis(≥50%) on CE-MRA in consensus.Another two experienced observers unaware of the result of CE-MRA evaluated significant stenosis in the three segments on NC-MRA independently.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of NC-MRA were calculated using CE-MRA as a standard reference.Results:All patients successfully underwent both NC-MRA and CE-MRA study.A total of 249 arterial segments were obtained in the 42 patients.The image quality of 243(98%) segments was diagnostic.Vein and soft tissue artifacts presented more frequently on NC-MRA but without obvious effects on the diagnosis.Significant stenosis was found in 87 of the 243 arterial segments on CE-MRA.The sensitivity,specificity,positive predictive value,and negative predictive value of NC-MRA for detecting significant stenosis were 93.08%,96.81%,94.17% and 96.21%,respectively.Intraobserver k-value was 0.91.Conclusion:Non-contrast MRA using bSSFP and F...
