Sir, NDM-5, a variant with increased carbapenemase activity in comparison with NDM-1, was first reported in China from an Escherichia coli strain followed by its report in the UK, India, Algeria and Japan.1–5 We report here the isolation of four NDM-5-positive E. coli strains responsible for various infections from four Chinese cities. The four patients, A1929, A2212, A2311 and A2509, lived in Zunhua, Chifeng, Hangzhou and Haikou, respectively, and have no history of hospitalization outside the cities they lived in. Patient A1929 was an elderly adult, while the other three patients were young adults, with or without comorbidities. Clinical features of the patients are summarized in Table 1. The periods between hospital admission and isolation of carbapenem-resistant E. coli varied from 0 to 16 days with the 0 day case probably a community-acquired infection and the other three cases probably hospital acquired. The four carbapenem-resistant E. coli strains, 1929, 2212, 2311 and 2509, isolated from the four patients from urine, blood, surgical wound swab and vaginal secretion samples, respectively, were deemed responsible for the patients' infections.
