Effect of combined therapy with phosphate and calcitonin on bone volume in osteoporosis

Twenty-six patients with postmenopausal osteoporosis were randomly divided into three treatment groups and were then treated for a period of six months with either : 1) double placebo ; II) oral placebo and parenteral calcitonin ; or III) oral phosphate and parenteral calcitonin. The study was carried out with a double blind protocol. Bone biopsies were obtained before and after the period of treatment and quantitative histomorphometry was performed on these biopsies by two independent observers. The results show that in patients in groups I and 11 there was no significant change In trabecular bone mass or In bone remodeling parameters. However, patients in group III showed a 36 percent increase in trabecular bone mass and an 85 percent increase in extent of bone formation surface without a significant Increase In either osteoclast count or extent of bone resorption surface. In treatment group III plasma immunoreactive PTH also rose significantly.
