Hip Strength and Core Endurance in Runners With and Without Iliotibial Band Syndrome

Iliotibial Band Syndrome (ITBS) is one of the main causes of lateral knee pain in runners. One etiological variable associated with ITBS is ipsilateral hip abductor weakness. Poor trunk control also contributes to pelvic drop and hip adduction, which may contribute to ITBS. The purposes of this study were to compare hip abductor and adductor strength and back extensor endurance, as a measure of trunk control, between runners with and without ITBS. Fifteen runners who averaged ³12 mi/week participated (ITBS: 4 females, 1 male; Controls: 6 females, 4 males). Hip abduction and adduction strength (N) was measured using a handheld dynamometer (HHD). Subjects were positioned in a supine position with the HHD on the distal lower leg. Each subject performed a 5s isometric hip abduction and adduction contraction. Subjects completed one practice trial and 3-4 maximal contraction trials. The three trials that fell within 10% of each other were averaged and normalized using body weight. Back extensor endurance (s) was measured with the subjects prone and the upper body unsupported. Subjects held their trunk parallel to the floor for one isometric contraction until failure. A 1-way MANOVA was used to examine differences in abduction and adduction strength between groups.
